What Forms of Payment Do You Take?
Cash, check, or charge (MasterCard and VISA).
Do You Take Insurance?
Yes. We’ll bill your insurance for you and make sure all your paperwork is submitted properly. We are contracted with United Health Care.Daily experience with claim forms and insurance companies has made our office staff members experts at spotting insurance company errors and obtaining prompt and maximum reimbursement payments.
What About Worker's Comp?
Provide us with the name of the insurance carrier handling your claim, the claim number, and the name and contact information of the claim adjuster. Weâ€™ll verify your insurance coverage prior to your second appointment, let you know if we need any additional information, and file your claim for you.
What should I bring on my first visit?
Please be on time for your first scheduled visit. A late arrival may cause a rescheduling of your appointment. Please bring the following items to your first visit:
- The referral or prescription written by your doctor
- Health insurance card or Workman’s Compensation info.
- My First Visit Paperwork
What type of clothing should I wear?
You should wear loose fitting clothing. For example, shorts, T-shirts, sweat pants, or athletic shoes.
Will the same physical therapist see me?
Our goal is to keep you with the same therapist so your treatment will be consistent which is more beneficial for a speedy recovery.
How long will my appointments be?
The first appointment is approximately 1 hour which includes an Initial Evaluation and treatment. Each subsequent appointment takes between 30 and 45 minutes.
